Qg5 forced the weakness on f6 and he just hampered in his king from there.
Black was very helpful. First he hands you a space advantage with 7...Nxe5? [instead of 7...Qc7]. Then, as ugly as it looks, I think he has to try 9...Kf8 rather than weaken the dark squares. The final nail was 12...d4?? which just handed over the e4 square, allowing the finish. I get the sense that Black was frustrated with his position and lashed out, trying to play a more confrontational move instead of meekly defending, but it only hastened the end.
