Looks like you played pretty well until the end game, but your critical mistake was 25. b5?.

If instead, 25. Qxa6, then 25. ... bxa6 26. Bxg7 Rd8 or Re8 (26. ... Rg8 27. Rxh7!) 27. c3 and it doesn't look like black can hang on the the h pawn. Then, white has a passed c pawn and has an excellent end game (not sure if it's a won end game, but it's pretty close).

You're right. 25. b5? was the blunder. 25. Qxa6 was the winning move.

To continue your line, I think that the best move for black would be 26. ...Re8. But after 27. c3 Re2 28.Bd4! he will be forced to either give up his bishop or lose almost all his pawns. Clearly a win for white.
