Loads of practise, analyse all your previous games especially ones where you lost, find where you went wrong and learn from those mistakes.
Books and chess software are a good source, learn your openings, the internet is a great source for self tutoring as well.
But like I said there is no substitute for practise.
Good luck in your search for perfection

The chess club idea is essential, i only started playing a year and a half ago but i found my strength as a player improved a lot once i joined a club. If you want to join one, im a member of Ealing chess club, don't know where u are in london but we're a friendly crowd and we could use some more young players.
If your interested, we meet on thursdays in The Harvester just opposite Boston Manor Station at 7.30pm. Maybe see you down there π¬~

Study the endgame rather than openings.May seem odd,but it really works.
I agree with the advise given earlier to join a club.
Play tons of games, bringin into practice what you've been studying. This may seem obvious but most people do NOT bring into practice what they studied.
Good luck!
