38...Ra1 looks as though it is met by 39.Ne3+ Kh3 (say) 40.Bxf3, when White is pretty much guranteed a win as the Ne3 stops Rf1+.

Yea, but after any king move, the mate threat is gone, so f4 is now doable, and the knight gets kicked right off e3.
Don't feel too bad. We all have one of those in our closet.
In attack mode, and way ahead materially, I had managed to pin and remove the worrisome bishop that was causing me headaches (and possible mating threats) ... only to overlook the other bishop... Aggghh... if only I had questioned his last pawn move, instead of being so greedy. 😕
