It means that you are to find a legal game that reaches the position in 22.5 full moves. A full move is a move by both White and Black - numbered in the same way as on a chess scoresheet. The ".5" means that only White has made his 23rd move. The stipulation could also be read as Position after White's 23rd move. How did the game go?
