If you are inactive on the site for over 30 days you will now lose your queue position.

does that include those at the head of the queue? i.e. when both participants for the next game have left the site before the game is started by either.

I do not think so. If a game is in progress then the normal timeout rules must apply. If both players in a game leave then Russ probably needs to intervene.
