It depends who manages it. The official approved medical establishment will manage it in the most expensive, profitable way possible, because its primary goal, by corporate law, is maximum profit. Under this system the worst outcome would be zero sickness - which would mean zero profit.

At a certain point the government has to step in.
As for personal responsibility, we all pay taxes. We vote for how the taxes are spent.
There is an international treaty on human rights that says we all have a right to healthcare.
So, it’s logical that taxes are spent on making sure that right is implemented.
