To increase 500 points after the provisional period you would need to win at least 16 games. It would more likely require 30 as you won't get 32 points for every game.

To increase 500 points at the beginning of your RHP career you could win (or even play) as few at 5 games.

A person who loses 5 times against a 2100 player will have a rating of 1700 under the provisional system.

What if they were timed out, surely that couldn't be as many points.

The provisional rating formula is the average of your individual game performances. Your game performance is defined as your opponent's rating if you draw, your opponent's rating + 400 if you win and your opponent's rating - 400 if you lose.

The manner (manor) in which you win/draw/lose do not change this formula. Therefore starting your first 5 games against a 2100 player and then being timed out in all of them will give you a rating of p1700.

If not what is the highest known ratings rise?
The simple answer to that question is that we don't bother keeping those kind of statistics. If you want to start monitoring the graphs of several thousand people - knock yourself out.
