For me, my family [a steadily depleting number of people, sadly] uses a nickname derived from my middle name. They have NEVER used my first name. This name is also used by any friends who were, at one time, way back when, in orbit of my family [i.e. childhood friends].

I have never ONCE introduced myself to anyone using that name [in a similar way to how I have never described myself as being English to anyone I've met].

No one I've met - except for one person who had some contact with my mum - after leaving home at the age of 18 uses that nickname and almost none of them are even aware of it.
