Technically speaking, as an idiot is a person of subnormal intelligence, the answer must be, no, most people are not idiots Otherwise 'an idiot' would be a person of normal intelligence and hence not an idiot....

I am not sure about the exact criteria, but yes, it was used as a medical term for people with severe mental retardation.
