For that matter we could do that locally and start an aggressive program of capital punishment and harvest organs wholesale. Perhaps starting with drunk drivers that kill people on the road. Guilt is immediately evident and indisputable. No trial, no improsonment, do not pass go, zap them with a taser, sedate them, followed by a trip to the hospital. You could keep the sedated until you have donor matches and remove their organs in a marathon transplant operation.
Organs could be auctioned to the highest bidder, you'd have to pay for the cost of logistics and doctors/medical staff somehow. Plus you don't really want the poor to live longer. The poor and the stupid are already over breeding anyway. In fact, if demand becomes high enough, the poor would be a great source for organs. They can't afford lawyers and who's really going to care if they lose one of their 10+ children, they're just going to have more anyway. You could even impregnate the poor with your own genetic material to maximize the chance that you'll have a compatible organ donor when you need it.
