I'm dead serious here. I just fell off one a couple of hours ago, and trashed my knee (still able to walk, somewhat).
When setting up a ladder, make sure it's secure and that you're not working above a safe (standing) height on it, where your center of balance is too high. Don't lean too far to one side or the other to reach work you're doing. Move the thing to where you can reach easily. Don't shim the thing up one side or the other if it's leaning to one side because of an uneven surface. Find another way.
What happened to me is that I didn't secure the bottom properly (on top of the hardtop of my pickup truck), the angle was leaned too far over, the ladder slipped backwards (lucky me at the top!) and I came crashing down from 15 feet up against the side of the house onto both the ladder and the wooden deck.
I know better. What a frickin' idiot.

I work with ladders every day of my life.
If your not safe every time, something bad happenning is an inevitability! If ur ladder isnt high enough get a bigger one, if your working on slopes, invest in a ladder leveller, it'll cost you 120 bucks but is well worth it. Dont set ladders up on wet wood(it will kick out), be wary of dirt(it will sink) piles of rocks will slide. Common sense is all it takes dude.

Im not sure where you can buy levellers I always had to special order them. they are worth the money though.
