The best is John Paciorek (brother of Tom and Jim Paciorek)
He played one game for the Houston Colt .45s (later the Astros) in 1963. He had three singles in three at bats, plus two walks. He scored four runs and had 3 RBIs. That gives him a 1.000 lifetime batting average and a 1.000 on base percentage. Five plate appearances and was never retired.

But it wasn't as short a Larry Yount's career (Robin Yount's brother). He made one appearace as a pitcher but was injured during his warmup throws and never played again. So he officially appeared in one game but never threw a single pitch.
