please tell me where both players went wrong except for moves 5...f6? and 38...Nb6??(for move 38 opponent only had 20 seconds left!) If you have any instructive games with these time controls please post them!

I saw that game. Well this one wasn't decided on a blunder but it sure made the game go by a lot quicker.
