3 possible explanations.

1. He moved to quickly and didn't see the blunder.
2. He was blindly following the book line and theory ended and he didn't know what to do. Lots of users do this, that's why you can't just follow book lines. You have to know what is going on.

3. He wouldn't be doing this but and i'm not accusing him but just throwing it out there:

Chess program crashed and he doesn't know how to play chess.

I think it was 1.

It's easy to hang a queen or something also if you are studying a very long line that wins material etc. and you overlook a piece of your opponents'.

But I agree, most likely it was #1
