Castling into the attack was not a great idea, and of course you should have exchanged queens on the many chances you had to do so.

You need to study tactics. You missed some very basic forks and discovered checks in the game. http://www.chesstactics.org/

For example, 17...Qxe3 18. fxe3 Nxf3 and then if he captures with 19. gxf3, you have a bishop fork with Bxf3, winning the rook.

Also, your opponent put himself in an awkward position with 18. Rxb7. You could have played a similar continuation as before, but just so you get the idea, I'll present a simpler solution (though not as good in this position), 18...Nxf3 (clearing the Kings file), 19. gxf3 0-0-0, checking your opponent and winning the rook with Kxb7. This isn't as good in this position, because you'll lose the bishop, but it's just to let you know that the king can be used as an attacking piece as well.

They call it chessblindness. Happens to everbody. I had it last week at my chessclub. I played quit well, but my 39th move was a typical "grave" move (in dutch we call it a grafzet). So I gave up! It's a hard but the best way to improve!

The best way to prevent this from happening is to study tactics. Get Chess Tactics for Beginners by Conveckta. I make 1/10th of the blunders I used to make(though I still make many) just by doing 50 puzzles per day.
