I would have thought that death was itself a consequence... and a pretty severe one too.
Consequences are the result of things, not the cause of things.
Thus blaming a 'lack of consequences' for causing anything is nonsensical.

To me the lack of consequences means that when a young person does
something wrong, he has received no spankings, restictions, or other
type of consequences for his actions for so long that he continues doing
worse and worse things until he does something so stupid it causes his
death.

Are you suggesting that young people who commit suicide may do so - to some degree, or in part - because they haven't been physically assaulted enough by adults?

I was talking about proper discipline so the child continually
learns correct behavior that would keep him safe instead of
running after a ball into the street and getting hit by a car or
some other dumb action that he might do because of lack of
proper discipline.
