Our rural hospitals are in crisis. Across the country, many small hospitals that serve remote communities are on the brink of collapse. They simply can’t survive on low patient volumes and high costs. Here in Vermont, nearly every hospital — except the state’s largest, located in our most densely populated area — is operating close to, or in, the red.
These hospitals are the backbone of their communities. They’re not just where people go in emergencies — they’re major employers and essential to rural life. When a city hospital closes, patients have options. When a rural hospital shuts its doors, the next one might be fifty miles away.
Earlier this year, the Republican-controlled Congress passed a major bill that included $50 billion for the Rural Health Transformation (RHT) Program, aimed at helping struggling rural hospitals modernize and survive. Many of these hospitals have outdated infrastructure, aging equipment, and shrinking staff. What works for urban hospitals often doesn’t work here.
Republicans fought to make sure this bill wasn’t just another bailout. Instead, the RHT program requires hospitals to design their own improvement plans — so federal funds go toward real solutions. The program allows hospitals to invest in technology for chronic care, expand opioid treatment, strengthen emergency services, or offer incentives to recruit staff who commit to rural communities.
It’s a commonsense plan that gives local hospitals control with financial support from Washington. But instead of getting behind it, Senators Peter Welch and Bernie Sanders are trying to strip this assistance away. In their efforts to posture during the government funding debate, they’ve proposed alternatives that would eliminate this program altogether.
Rather than supporting rural hospitals, Sanders, Welch, and Washington Democrats want to keep funneling money into the broken Affordable Care Act. That approach has failed for a decade. It doesn’t fix the system — it just kicks the can down the road. Costs keep rising, subsidies keep growing, and taxpayers keep paying.
If we follow that path, Vermont’s rural hospitals will continue to struggle or disappear. The only way to actually lower health care costs is to address the cost of delivery, not just how we pay for it. Insurance companies and federal regulators aren’t equipped to do that — but our hospitals are.
The RHT program puts decision-making where it belongs: in local hands. It rewards hospitals that innovate and collaborate. It’s about improving care, reducing waste, and making rural health sustainable again.
Unfortunately, Democrats in the Senate are blocking this progress. If Vermont’s senators truly care about rural communities, they should stop holding up the money needed to modernize facilities, recruit medical professionals, and deliver affordable care to families across the state.
The House has already passed a plan to reopen the government that protects this program. The Senate should follow suit. Vermonters can’t afford more gridlock.
Our rural hospitals are more than line items in a budget — they’re lifelines for their communities. They bring babies into the world, stabilize trauma patients, and provide comfort in life’s hardest moments. It’s time for Senators Sanders and Welch to stop playing politics and start helping Vermont’s hospitals do what they do best: care for their people.
