San Andreas had the pimping up, you know, the tattoos and stuff... GTAIV is way superior to anything made before, but it's purely improving the already existing ideas.

The Driving!
Absolutely spoilt the game for me did the driving. Mainly because I had to use the brakes...
Seems picky I know, it was otherwise an excellent game, but I just never got the fun out of it that I did in either Vice City or San Andreas, I could go exploring in VC or SA for hours, just listening to the radio cruising along at top speed with a couple of thousand police on my tail with no problem. Corner coming up, no problem, tap brake and power slide into the corner, plenty of time to correct steering before ploughing into that bus, and on we go....
Try that in GTAIV, *ok accelerate away into traffic, adjust camera so that it is pointing in the right direction as I need to see where I am going, Slowly build up speed, Do best to decipher GPS Route, ignore after crashing because you couldn't tell if the road was green or yellow, correct steering after crash, release accelerator to correct steering without fishtailing, get up to top speed listening to mediocre radio with a couple of thousand police on your tail, Corner coming up, start braking, lose control of car and pray that is holds a reasonably straight line, retune radio as you haven't heard anything funny from Lazlo in ages, Go make yourself a cup of tea and some toast, then finally, turn into the corner when you are going slow enough not to crash into everything. Rinse and repeat from the *
