This is how crossword clues work - cryptic ones.
The clue as given should usually contain a definition and some wordplay.
Previous clue was
Doctor treating the sick a bit.
the answer was Drill.
Doctor = DR ... the sick = ILL
DR+ILL = bit (as in a drill bit)
