Did you know the US Navy will not permit any country to land goods in Puerto Rico? It's because of some stupid law* where it's illegal for non-US built and owned ships to traverse internal US waters and because the PR is a US territory the Navy has decided this includes the waters around Puerto Rico.

So what has to happen is goods have to be dropped off on the mainland and then transferred to a US ship which goes back out to sea to the island.

This blockade is a major problem for the island.

*EDIT It's called the Jones Act and I'm oversimplifying but still
