Many times, but this one always cracks me up - I was playing a friend who is equally defensive, and our style of play is so similar that we bore ourselves to death and play almost without paying attention to each other's moves, they're so predictable. In this case, the mechanical approach went on for three half-moves.

My move 20 and 23 both blundered a piece; this was a terrible game for me that I ended up winning. Move 20 I simply placed my knight on an attacked square, but it wasn't taken. On move 23, I blundered another piece (more subtly this time) and again he didn't take advantage. If he'd played 23... Ne5, the queen would be attacked, but couldn't move anywhere that'd keep defending my knight.
