For me? Probably "Kids", "Henry, Portrait of a Serial Killer", "Man Bites Dog", "Killing Zoe", "Happiness"...I'll need to think of a few more.
I have never seen it and I want to but I guess "Last House on the Left" is pretty disturbing. I guess the special effects are so bad that it actually looks like a GD snuff film.

The Killing Fields definitely IS disturbing! (and really good too)

On a smaller scale I found the movie Festen pretty disturbing (English title is "The Celebration"😉, that one is about sexual abuse in a family.

Very good movie. The problem I had with it is that they didn't make the protagonist very sympathetic at all.

Forest Whitaker was great in that too.
