Most people are so wrapped up in their own little world that they haven't stopped to ask themselves this question. Some people are in 'instant activation mode', as if they were playing a game. They simply respond to the stimulus around them.
Everyone need to stop the clock, look at their life and decide what it is that they really want. Is it wealth? Sex? Health?
