Your 12...exf4 wasn't bad, but I might have played 12...Ng4 instead, which takes advantage of the fact that the white rook can't move, due to the possible Nf3 fork of Q and K. Also, the Ng4 move opens up a line of attack for your bishop.
The only other major tactic that you missed was 15...Qh1#, but that would only have shortened the game by a few moves.
