Also when returning home from a holiday he doesn't use the stairs.

I've not seen the bit about the rain before but it is an old one as you say.
I think Pat could also be called Warwick Davies - the actor

Jane is in a room at work. Having finished her work for the moment she turns off the light and walks out of the room.

Suddenly there is a loud CRASH from outside. Jane runs back into the room and looks out the window. Because of what she sees outside, she leaps out of the window to her death.

This must be an old teaser because I think all lighthouses are automatic these days.
Anyway, I suppose the answer is when she leaves she turns out the wrong light(was that ever possible?),a ship crashes into the lighthouse so it's tumbling into the sea so she jumps off it to try and save herself but cracks her head open on the rocks.
All highly unlikely of course but then these teasers often are.
