They really do have a free market economy for the most part, but what about population? India just landed a robot on the moon recently. 2nd most populated country on the planet. That is a factor too. Capitalism isn't just about having wealth it's about the drive to attain or increase it.

Capitalism has risks and rewards. Unfortunately, Congress, both the GOP and Dems, have allowed large corporations to avoid large risks but reap rewards. That's welfare for corporations.

Healthcare is a little different from most services by nature, and so has to be treated a bit differently. But the fundamental principle is the same.
That's because the consumer isn't the only one that controls markets; the businesses themselves also manipulate markets in their favor. This is why monopoly and labor laws are needed to do what free markets can't.
