Imagine a world where healthcare is... neither of those two things.

With private, third party payers paying for most things (who can just raise premiums), providers can raise prices because there is no real resistance to it."
In such a system, prices can rise LITERALLY without bound. And so they do. π

Biden capping the price of insulin was low-hanging fruit. The embarrassment that a life-saving drug over 100 years old would still cost so much PROVED the system was compromised by crony-capitalists. They couldn't pass the red-face test in front of the public, so they caved. One small victory.
ACA was similarly meant not only to expand coverage but to increase competition among private insurers - which in theory would drive down prices. Did it work? Probably a little. But insurance is not the main driver of these costs.
