You're getting there, I've done some research into answer 4(I answered no) but technically it could be yes though not apparent in all the one's I've seen!!

My sources leave only the toads and caecilians but one source says toads are a kind of frog. But both look like chordates to me. So I’m out of guesses.

It's the chordate thing that's put you off track I think.
I thought you were there with 14 when I answered "yes" to do you undergo metamorphosis but then you lost it a bit again.
Perhaps a re wording of 14 would give you the answer.
Sorry about the chordate clue but when you see the answer you'll realise why I said no

You are not - warm-blooded, reptile, insect, fish, shellfish, spider, worm, beetle, frog, bird.
You may be - a freshwater-dwelling species, a species which can fly (but not a bird or butterfly), a toad.

I obviously new a tadpole would eventually develop a backbone but not at the tadpole stage which is why I answered no to the chordata question.
Consider if the animal was a human embryo and the question was "do you have eyes"? wouldn't the answer be "no" ?
